Jose Baez, M.D.

Jose Baez, M.D.

Dr. Jose Baez is a Board-Certified General Surgeon with Board Certification in the sub-specialty of Surgery of the Hand. He is well experienced in micro-vascular surgery and minimally invasive carpal tunnel techniques. He is committed to providing experienced care and knowledge to his patients so that they can return to a normal and active lifestyle.
In addition to his position with the Hand and Wrist Center, Dr. Baez also provides care to hand and upper extremity trauma patients at a local Jacksonville trauma center, and sits on the medical executive committee of an ambulatory surgical center. He is also a member of the American Society for Surgery of the Hand, the Florida Orthopedic Society and the Clay County Medical Society.
Dr. Baez completed his undergraduate education at the University of Florida, and then returned to his native Puerto Rico where he attended Universidad Central del Caribe Medical School. He completed his general surgery residency at The State University of New York Health Science Center (also known as SUNY Downstate Medical Center) in Brooklyn, New York. Dr. Baez then completed his hand surgery fellowship at the University of Miami Jackson-Memorial Hospital.
Now practicing at the Hand and Wrist Center, Dr. Baez focuses on the diagnosis and treatment of both surgical and non-surgical conditions of the hand, wrist and elbow. Dr. Baez is fluent in both English and Spanish.

Richard D. Curtis, M.D.

A Board Certified surgeon, Dr. Curtis specializes in hand surgery and helping patients recover from hand injuries.
Born in Tokyo, Japan and raised in San Antonio, Texas, Richard D. Curtis, MD, FACS studied medicine at the University of Texas Medical School in Houston. He completed Residency at the National Naval Medical Center in Bethesda, Maryland and Fellowship in Surgery of the Hand at Baylor College of Medicine with special interest in Dupuytren’s Disease.”

Dr. Riley Schlub

Dr. Riley Schlub is a board eligible plastic and reconstructive surgeon who is fellowship trained in hand and upper extremity surgery. 

Originally from Ohio, Dr. Schlub obtained his undergraduate degree at Rollins College in Winter Park, Florida and doctorate of medicine at the University of Central Florida College of Medicine. Dr. Schlub briefly left the Sunshine State for Columbia, South Carolina, completing residency training in plastic and reconstructive surgery at the University of South Carolina. Following residency, he returned to Florida to obtain fellowship training in hand and upper extremity surgery from the Florida Orthopaedic Institute in Tampa.

Dr. Schlub sees a wide variety of patients not only with surgical and nonsurgical hand and upper extremity conditions, but also with reconstructive concerns. He has a strong interest in microsurgery, particularly peripheral nerve surgery. Additionally, he enjoys treating a wide variety of upper extremity fractures, ligamentous and tendinous injuries and arthritic conditions. He also sees patients for traumatic and oncologic reconstruction, facial fractures as well as body contouring procedures

Jackie Nizamoff, OT/L, CHT

Jackie Nizamoff,
OT/L, CHT

Ms. Jackie Nizamoff is a Certified Hand Therapist with the Hand & Wrist Center and brings over 30 years of experience to her patients.
Originally from Indiana, Ms. Nizamoff graduated with her degree in Occupational Therapy from Indiana University in 1989. In 1994 she became a Certified Hand Therapist, focusing her practice on the care of hand and upper extremity clients. Ms. Nizamoff has extensive experience in fabrication of custom orthotics for the upper extremity, conservative management, and postoperative care of simple to complicated shoulder, elbow, wrist and hand injuries.
